To request a backup of an existing SQL Server database, please submit the following Cherwell form: 

https://service.uiowa.edu/CherwellPortal/ESCPortal/One-Step/Backupdatabase

Need help filling out the form? Please refer to the below:

1. Name of the server where the database resides
2. Name of the database to be backed up
3. If the database must be created by a certain date, please type in the date (and/or time)
4. If you would like a separate notification email once the database is created, enter the email address here
5. Any additional requests or needs for the database backup
