If you are a current or former student:

Your 'official' name is listed on your registration at the University of Iowa. You may check with the Registrar's Office to verify how you are registered. You can change your 'official' University name at the Registrar's Office. Name change requests require completion of "The University of Iowa Name Change Form" available on the Registrar's web site.

NOTE:

Graduate students writing a thesis are required to use their 'official' University name in their Preliminary Pages. The name shown on the Preliminary Pages must match their recorded name with the Registrar's Office.

If a graduate student does not wish to submit their thesis using their 'given name' (an example would be First Name - Middle Initial - Last Name), he or she must request a name change to the Registrar's records before submitting a thesis to the Graduate College.

Faculty & Staff:

If you have been a student at the University of Iowa:

All faculty and staff who are former or current students are required to change their "official" name through the Registrar's Office. Name change requests require completion of "The University of Iowa Name Change Form" available on the Registrar's web site.
NOTE: Any name changes done through the Employee Self-Service site will be changed back to the "official" name on file with the Registrar's Office if the Request for Change of Name form and appropriate documentation is not submitted to the Registrar's Office.  

If you have never been a student at the University of Iowa:

All faculty and staff who have never taken any courses at the University of Iowa can change their "official" name via the Employee Self-Service  site under
"Name/Address/Phone Changes."
